Small Business Life Insurance | Accidental Death and Dismemberment Insurance
This plan’s life insurance coverage pays benefits if a covered employee dies from any cause. 100% benefit is paid for loss of both hands, both feet, loss of vision in both eyes, or any combination of these losses. 100% of the AD&D benefit is paid for loss of speech and hearing, and also for quadriplegia. 75% of the benefit is paid for paraplegia. 50% of the benefit is paid for loss of one hand, foot or eye, loss of speech, loss of hearing, or hemiplegia. 25% of the benefit is paid for loss of thumb and index finger on same hand or uniplegia.

AD&D coverage also includes the following benefits, which are paid in addition to the life and AD&D benefit: seatbelt, airbag, repatriation, dependent education and coma.
You may choose to provide small business life and AD&D insurance equal to:
- Any multiple of $5,000 subject to minimum of $10,000
- Any multiple of annual base earnings (for example, 1, 1 ½ , 2, 2 ½, etc.)
- A specified amount based on job class (up to three classes) and the size of the group, subject to a minimum of $10,000 for any class
An employee’s life insurance coverage will be reduced by 50 percent at age 70, and further reduced to 25 percent of the original benefit amount at age 75. In addition, you may choose to provide coverage for eligible employees as of the date of hire or after a one, two or three-month waiting period.
