About the Program
COSE understands the unique needs of small business. The COSE Dental Program offers a variety of plans at varying cost points to accommodate even the smallest benefit budgets. Five distinct dental plan options offer quality plan coverages from a well-respected regional insurance carrier, Medical Mutual of Ohio.

Five Plans to Choose From
The COSE Dental Program includes the Basic PPO, Intermediate PPO, Advanced PPO, Alternative Basic PPO and Alternative Advanced PPO. All of the plans cover preventive services at some level (85 percent or higher) with no deductible.

The Alternative Basic and Advanced PPOs cover Essential Services (fillings, root canals, extractions, impactions, diagnostic x-rays) at 50 percent or 100 percent, respectively. The cost for the Alternative PPO plans is slightly higher because there is no deductible for such services. Additionally, these plans both cover a portion of orthodontia and complex services (crowns, bridges, dentures, etc.).

Participation And Contribution Requirements
The Advanced PPO and Alternative Advanced PPO both require a minimum of 50 percent employee participation and 50 percent employer premium contribution. The Basic PPO, Intermediate PPO and Alternative Basic PPO have no minimum participation requirement and no employer premium contribution requirement.

COSE understands that employers may not always choose to contribute to an employee dental program – that is why COSE offers three fully voluntary (100 percent employee-paid) dental options that also require no minimum participation by employees.

Network Access
The COSE Dental Program is offered through one of the nation’s leading dental PPO networks – DenteMax. The DenteMax network consists of over 70,000 independent dentists who agree to become part of the DenteMax network and therefore accept a fixed, lower rate for dental services, typically 25 to 40 percent below their usual charges. With DenteMax, COSE members have national access to a leading credentialed dental PPO. To learn whether your dentist participates, log on to www.DenteMax.com .

Freedom of Choice
The COSE Dental Program offers members freedom of choice to visit dentists both in and out of the network, although visiting an in-network dentist will provide the highest coverage and best value. Additionally, participants in the program can select a new dentist at any time, and each family member can select his or her own dentist.
