Medical Mutual offers four distinctly different ‘families of products’ all designed to meet the varying needs of smaller employers.

HMO Health Ohio Product 1 Select any HMO Health Ohio primary care physician to coordinate all of your health care services. Select a hospital from the HMO health Ohio network.

Employees who currently have or will select a physician in the HMO Health Ohio network. 

 

Employees who like a more ‘managed’ benefit program.

 

Referrals are required in order to visit specialists.  Members can change physicians at any time, but must notify MMO prior to visiting new physician.

Medical Mutual Eligibility Requirements

For groups enrolling on or after July 1, 2006, companies must meet a minimum participation requirement of two enrolled employees. For example, a group with two eligle employees must maintain the two enrolled contracts to remain eligible for coverage.

Net eligible active employees do not include full-time employees who are enrolled in: 1) any other Medical Mutual group health plan, or 2) their spouse’s employer-sponsored group health plan coverage.

Medical Mutual will continue to allow for sole proprietor coverage, meaning only 1 enrolled contract or 1 enrolled employee in the group; however, in order to be a legitimate sole proprietor group, the 1 enrolled employee must be the only eligible employee at your company.

If a group enrolls less that 50 percent of the total active eligible, Medicla Mutual will require a completed medical questionnaire on each waiving employee.

































Who Is Eligible?
Only full-time employees are eligible to enroll in a COSE plan.

Full-time employees are officers, partners and employees of your company who you classify as full-time.

For employees working between 17.5 hours and 25 hours per week, you, as the employer, are permitted to choose the threshold at which an employee is considered full-time.

For example, you can decide that anyone working 20 hours a week is full-time. MMO or Kaiser will consider these employees eligible for health insurance. Remember, however, that you need to be consistent and non-discriminatory in applying this to your workforce. Once you decide what your full-time threshold is, everyone working that number of hours per week will be considered full-time. Employees working more than 25 hours a week will automatically be considered full-time.

Independent contractors (those who compensation you report on a 1099 form) are not considered eligible employees for your company’s health insurance program. An independent contractor may apply for group insurance under his/her own policy.

Employer Premium Contribution
As an employer, you must contribute at least 25 percent of the total health insurance premium for each employee and retiree enrolled in a Medical Mutual plan. Your company’s application may be requested if it does not meet Medical mutual guidelines.
